Group health plans that offer self-insured minimum essential coverage must provide participants with Form 1095-B, documenting enrollment in plan coverage, and file all such forms with the IRS.

This requirement includes:

• • Group health plans (including those offered by small employers)
  • Multi-employer plans (including those offered by small employers)

Form 1095-B is used to report certain information to the IRS and to taxpayers about individuals who are covered by minimum essential coverage and therefore are not liable for the individual shared responsibility payment.
